The cover is moderately worn and soiled, edges chipped. There's a row of dried old adhesive near the top of the front cover. Paper is yellowed, particularly near the edges. Center page has separated from the top staple. Text is unmarked.
Included is a 8-page pamphlet, "Forest Lawn Serves the Living." This smaller advertising piece, published in 1957, has a keyed diagram of Jan Styka's "The Crucifixion" as the centerfold.
